WEBSITE USE AND CONFIDENTIALITY AGREEMENT
Medicaid Quality Performance Use and Confidentiality Agreement
The Institute for Child Health Policy (ICHP), which administers the Medicaid Quality
Performance (MQP) website, may change this agreement at any time. Continued use of the
website indicates acceptance of the most current agreement. Any changes to this agreement
will prompt reacceptance by the User. It is the User’s responsibility to remain informed
about these changes.
What the User Should Know
The information provided on the MQP website is for general informational purposes. ICHP
is not liable for any damages arising from the use of this website or linked sites,
including data loss or service interruptions. Content on the website may be changed or
updated at any time without notice.
User Agreement with ICHP
Use
The website is for official state-approved business only. Use for personal purposes is
prohibited. Users must not violate any laws or regulations applicable in their jurisdiction.
Privacy and Confidentiality
Information on the website is confidential and may not be shared outside of authorized
organizations. Confidential credentials (Usernames and Passwords) must be safeguarded. If
compromised, the User must notify ICHP and their organization’s administrator immediately.
Suspected fraud or security threats must be reported immediately.
Personal Security Identification Codes
Users must not share their ID and Password. They are responsible for actions taken under
their credentials, including any unauthorized access caused by negligence. All transactions
under a User’s credentials are considered legally authorized by that User.
Access to Data
Users must have proper authorization to access data. Attempts to access or alter
unauthorized data are prohibited. Preventing access to authorized users is also not allowed.
Violations will result in permanent loss of access privileges.
Notice of Non-Liability
ICHP does not accept responsibility for User hardware or Internet issues. It makes no
guarantees about the accuracy or availability of the content and disclaims liability for any
resulting decisions or damages. Users should contact
mqpadmin@ichp.ufl.edu for any questions.
ICHP may deny access to anyone at any time and reserves the right to monitor activity,
inspect transmitted files, and manage content without prior disclosure.
Confidentiality Agreement with ICHP
By using the MQP website, Users agree to keep all information—written, oral, or
electronic—confidential. Information may be shared only within the limits of applicable law.
Consent to Electronic Transactions
Electronic transactions are legally binding. User consent via credentials is equivalent to a
written signature. Users are responsible for clearing their Internet cache to prevent
unauthorized access to PHI.
The agreement may be terminated if the User cannot meet technical requirements necessary for
secure electronic communication.
